The Challenge
Along with hereditary levels of Chronic Conditions such as asthma
and diabetes, the lifestyle of children is becoming more
sedentary, while their diets are becoming higher in fats and
calories. Obesity problems are projected to drive down our
children's life expectancy. Disease Management companies and
Specialists handle a portion of these patients, however,
Pediatricians are still the frontline when it comes to Chronic
Condition Management for children.

In contrast to Integrated Models, where
the issue is largely around detection of under-diagnosed
conditions, Chronic Conditions generally are already known, but
need to be managed. To do this, the easiest approach is to
mirror the practice's treatment protocols using registries.
Within each registry, data collection protocols are further
defined, assigning specific questions and measures (adherence,
severity, co-morbid psych, quality-of-life, etc) to different time
intervals (phases) used in each protocol.
Based on the child's known Chronic Conditions, the
associated data collection protocol and trigger logic, the Electronic
Interview automatically collects the required patient information.
Integrating
the data into the child's treatment, provides Pediatricians
with key information, as well as information they normally would not
have the time to gather, thus making them more effective in
the management of the child's Chronic Condition(s).
Looking at Outcomes
Using Patient Tools to manage the data collection process
allows for an instant aggregate view of how well your patient population
is doing. Practices/providers are able to access and view
how the Chronic Condition treatment protocols are working
on average, compared to norms or set points. In addition,
providers are able to drill down to see how well sub-groups
are doing or look at individual cases.
